Our Services

  • This service occurs annually within the guidelines provided from the FSSA, BDS, and Federal requirements. This meeting is to help individuals and their providers plan for expected services, renew previous services, and change plan outcomes based on needs. The meeting is in person, in the home of the individual and will be scheduled with your Case Manager.

  • Once per quarter, your Case Manager will need to schedule an in-home visit to ensure that services are being delivered and remain appropriate. This appointment is a monitoring appointment with mild paperwork requirements

  • Once monthly, your assigned Case Manager will check in briefly to ensure the delivery of services, schedule upcoming appointments, ensure that there are no discrepancies in your care.

  • Your assigned Case Manager will review your documents and request new documents regularly as it pertains to your Medicaid Waiver Services. Case Managers can also do three way calls with providers to assist in certain instances with advocacy.
